Sobczak Wencel Dentistry also serves as a referral office for Local Dentists. We are happy to coordinate with your General Dentist to provide limited services such as Implant Placement, Wisdom Tooth and other Extractions and Partial / Complete Denture fabricaiton.
If you would like more information regarding our referal services and reasonable fees, please print out our referral form and have your Family Dentist complete it. We will then schedule a consultation appointment. Should you decide to pursue referral treatment with us, make sure your Family Dentist indicates the extent of treatment he or she desires for us to complete. Your Dentist may want us to place an implant for you and then return to their office for them to complete the restoration with an abutment and crown. You will be returned to your Family Dentist for your other continuing treatment needs.
